I. Dasar Hukum
Peraturan Bupati Pakpak Bharat Nomor 15 Tahun 2009 tentang Pendelegasian Sebagian Wewenang Pengurusan Perizinan dan Non Perizinan Kepada Kepala Kantor Pelayanan Perizinan Satu Pintu dan Penanaman Modal Kabupaten Pakpak Bharat.
II. Persyaratan
Persyaratan Izin Baru dan Perpanjangan adalah mengisi formulir permohonan dengan melampirkan :
1) Fotokopi bukti kepemilikan hak atas tanah (fotokopi alas titel/hak alas tanah) yang telah dilegalisir oleh pejabat yang berwenang.
2) Sket lokasi tanah milik dengan skala 1 : 5.000.
3) Fotokopi Kartu Tanda Penduduk (KTP) pemohon.
4) Pasfoto ukuran 3 x 4 cm.
5) Berita acara pemeriksaan letak dan keadaan lapangan serta potensi kayu yang menyangkut jenis, tinggi, diameter dan volume kayu yang dilaksanakan oleh tim teknis yang terkait.
6) Surat rekomendasi dari Instansi terkait.
III. Retribusi
Retribusi untuk izin pemanfaatan kayu rakyat adalah Rp. 0,- (Nol Rupiah,-).
IV. Waktu Penyelesaian
Waktu penyelesaian izin paling lama 21 (dua puluh satu) hari kerja setelah berkas diterima dengan lengkap yaitu paling lama 14 (empat belas) hari kerja untuk survei lapangan dan paling lama 7 (tujuh) hari kerja untuk penerbitan izin.
V. Masa Berlaku Izin
Masa berlaku izin adalah paling lama 6 (enam) bulan dan dapat diperpanjang kembali paling lama 6 (enam) bulan lagi.
